Wondering if there is a way to verify the form creator. I know Google Forms has this, but it is also linked directly to the website domain, where Fibery is not. Unsure how this can work, but I can right now make a form and say it is from Nike, but it is actually a scam. Maybe verify domain via dns? Not urgent, but something to think about.
